Population Health Project

The Ohio AAP/Ohio MCO Population Health Project – is a clinic-tailored quality improvement project driven by a partnership between the Ohio AAP and Ohio’s Medicaid Managed Care Organizations to pilot small tests of change in pediatric primary care practices to impact well care and immunizations in a pediatric population. By bringing together intervention ideas, education, and resources from primary care experts, content experts, and payors, innovative interventions and strategies to remove barriers that typically get in the way of improving well care and immunizations for the pediatric population are addressed. Improved preventative care for Ohio’s children leads to better health outcomes and improved quality of life.
