Are you a provider interested in asking moms, “Care to talk about…your health, too?”
The Care2 QI Project will launch over the next few weeks. This new approach will combine lessons learned in the Smoke Free Families and Healthy Mom, Healthy Family project into a two generational care approach. The project will be implemented in family medicine practices from September 2023 – August 2024, providing education, discussion, and resources for children birth – five served by these practices. Topics of focus in the project will include:
- Smoke exposure and cessation of tobacco or vape use
- Family planning and multivitamin use
- Safe sleep environments
- Maternal depression
Participants in Care2 will:
- Connect with mothers during well-child visits to identify risk factors and connect them to appropriate resources and treatment
- As she considers future pregnancies, use valuable in-office time to consider mom’s health, too
- Utilize enhanced protocols to improve office processes related to interconception care

QI Project
Practices who provide well-child and adult care will be eligible to apply for participation. Participants in the 12 month QI project will receive:
- A $3,000 stipend to support data collection
- No chart review! Utilize EHR data to produce site-specific and aggregate QI data accessed on a digital dashboard
- Complimentary materials to improve discussion of maternal health in your practice
- Support from a dedicated Quality Improvement Practice Coach
- Learning collaborative to share challenges and success with peers across Ohio
- Provider MOC Part IV credit upon completion of QI activities
This project is funded by the Ohio Department of Health and Ohio Department of Medicaid and supported by the Ohio Colleges of Medicine Government Resource Center.
