If you are a clinic administrator for primary care providers who treat children and adolescents, fill out our Interest Form today! After submitting the Interest Form, you will receive the OPPAL Participation Agreement via email within 24 hours. Once the Participation Agreement is completed, clinic administrators will be able to enroll all the providers associated with the clinic. Please submit one interest form per clinic.
