Providers Enrolled in OPPAL
- Only providers enrolled in OPPAL with a Trayt Health account may use the consultation e-request platform.
- If you are enrolled and have issues accessing your account, try resetting your password or contact support@trayt.health.
Providers Not Enrolled in OPPAL
Providers not yet enrolled in OPPAL can enroll using one of the following options:
- Call the OPPAL phone line at 614-605-1511. By calling the line, you can enroll and request a consultation at the same time.
- Complete the OPPAL Interest Form. After submitting the Interest Form, you will receive the OPPAL Participation Agreement via email within 24 hours and a separate link to complete the PCP portal registration.
