Providing high quality, evidence-informed adolescent health care requires planning, teamwork and precision, all with a focus on maintaining patient confidentiality. For those who provide care routinely to adolescents, we recognize how valuable the experiences and tools of others can be in helping us improve our clinical practice and care. Those clinical tools include things such as medical note templates that maximize pertinent screening, clinic processes that address preventive care needs and clinic policies or practices that help assure patient confidentiality — just to name a few.
The goals of collecting and organizing these clinical tools and processes are to:
- Give you access to clinic-tested tools to enhance adolescent care in your own clinic
- Share those adolescent-informed clinical practices with the primary care community
- Improve health care for adolescents wherever they are seen
Please submit your adolescent clinical care tools and documents via the form below. Once you have submitted your documents, they will be uploaded and organized on a secure Ohio AAP database site. You will receive an email from the Ohio AAP acknowledging their receipt and giving you the password to access the entire collection of resources for your own clinical practice as well.
Please keep in mind that these documents should NOT include any patient-specific information. These documents should be BLANK note templates or clinical processes that can be shared with other medical providers to enhance care in our outpatient and inpatient settings.
