The Ohio AAP is committed to addressing both health disparities and infant mortality through two projects kicking off in early 2021. We invite you to be a part of our movement to ensure that all babies in Ohio are given the best opportunity for a healthy start.
Below are details on how you can become involved – each project offers benefits for you and your patients, including:
- -FREE family resources, including books, multivitamins, cabinet locks & more!
- -MOC Part IV Credit from the ABP and opportunities for CME or MOC Part II credit
Injury Prevention Plus SEEK Quality Improvement Collaborative
- Electronic screening tool to identify and address social determinant of health and injury risks for families birth to five years of age
- Earn 25 Points of MOC Part IV credit in participation from February – June 2021
- Receive books and cabinet locks to provide to families in your practice, as well as connections to local community referrals

For a more in-depth introduction and a chance to learn more, please consider joining the January 27th virtual Central Ohio Pediatric Society’s (COPS) January meeting for a presentation on “Social Determinants of Health and Practical Applications for Counseling in the Primary Care Setting.” presented by Dr. Mike Gittelman. Healthy Mom, Healthy Family Learning Collaborative
- Innovative approach to addressing maternal health in well-child visits for birth to 18 months of age
- Easy data collection tool to reduce chart reviews
- Up to $1,000 in resources for your patients – like multivitamins, board books, technology assistance, and printed materials
- Earn up to 50 points of MOC Part IV credit in 2021 by participating in the full 12 month program beginning in February
