More than 40 people gathered in Columbus this week to discuss an exciting, new partnership between the Ohio Chapter, American Academy of Pediatrics and the Pathways Community HUB sites in Ohio.
Six organizations across Ohio currently serve as Pathways Community HUBs, which are extremely connected groups that serve to identify at-risk families in their region. Community Health workers conduct home visits, screen the families and work with them to improve outcomes. The current Community HUBs in Ohio are located in:
In addition, other organizations are hoping to receive HUB certification in Cleveland, Dayton and Athens.
Mark Redding, MD, FAAP, and Sarah Redding, MD, MPH, co-developed this now nationally-utilized model. The Ohio AAP, led by President-Elect, Mike Gittelman, MD, FAAP, is working with both Drs. Redding along with the leaders at each Community HUB to create set curriculum and provide resources around immunizations. In addition, the groups will continue to work together to do the same around obesity prevention, safe sleep, and other topics.
