The purpose of this survey is to determine Ohio providers’ unique barriers to childhood vaccination, especially with the fourth dose of the DTaP (diphtheria, tetanus, acellular pertussis) vaccine. This vaccine is one of Ohio’s lowest rates in the childhood vaccination schedule. The National Immunization Survey and AFIX data indicate between 73.5-80.9% of Ohio two-year-olds received their fourth dose in 2015. The Health People 2020 goal for all childhood vaccines is 90%!
The Ohio AAP and Ohio Department of Health (ODH) partnered to raise awareness about the DTaP vaccine and educate providers through a series of presentations on strategies to raise the rate of the fourth dose through reminder and recall (see date/time and registration information below). The information you provide in this survey will be used to develop these presentations!
Completion of the survey is a requirement to receive American Board of Pediatrics (ABP) MOC Part 2 and AMA Category 1 CME credit for the activity. The survey will take approximately 5-7 minutes to complete and your responses will be strictly confidential. The final question will ask for your name, although required for those providers seeking credit, we are hoping some of you might be willing to share additional information about your practice’s experiences at some point. In an effort to measure the efficacy of this program, pre-post evaluations will be studied along with impact in the months following the activity.

Successful completion of this CME activity, which includes participation in the activity, with individual assessments of the participant and feedback to the participant, enables the participant to earn 3 (three) MOC point in the American Board of Pediatrics’ (ABP) Maintenance of Certification (MOC) program.
It is the CME activity provider’s responsibility to submit participant completion information to ACCME for the purpose of granting ABP MOC credit. MOC Part II credit will be entered into the CME data portal entitled PARS and will be shared electronically with the ABP within 30 days of the activity date.
