Title: Obesity Prevention in Primary Care

Description: The Obesity Prevention in Primary Care self-assessment contains 50 questions that address childhood obesity. Childhood obesity is now recognized as a public health priority and primary prevention is a critical component of a sustainable solution. Pediatricians, as primary care health providers play a key role in prevention as a reliable source of health advice as well as experts in developmentally appropriate approaches to prevention. A critical window of opportunity exists through out the prenatal period into the first 5 years of life. This selection of articles support new research and strategies to enhance prevention efforts within the pediatric clinic setting. Healthy weights can be achieved by assisting families in health promotion by building a fundamental foundation of good nutrition and activity early in life.

Completion Criteria: Please allow for 2 to 3 hours to read the reference material and answer the questions. Once you begin the assessment, you have until the end of the calendar year to complete it, but are not required to complete it in one sitting. Please note: answers may not save when exiting an incomplete assessment; if concerned about ability to return, we recommend that you keep a list of the answers that your provided to enter later should your answers not save. You must answer all of the items AND answer 80% of them correctly in order to receive credit for this assessment.

Credit Assignment Criteria: Upon completion of the activity, you will be required to enter your American Board of Pediatrics (ABP) Diplomate number*, state and birthdate (MM/DD). Your ABP Diplomate number can be found in your ABP online profile. 2015 MOC Part II Self-Assessment – Obesity Prevention in Primary Care

This course is designed to provide resources to pediatric practices to help counsel families of infants and young children (ages birth to 5 years) about good nutrition and positive parent-child interactions during mealtime and playtime to encourage their child’s development.  This Quality Improvement program provides tools for physicians to enhance prevention counseling strategies, become adept at assessing “risk,” and intervene at the earliest possible stage before a child is overweight.

Target Audience: The target audience is pediatric healthcare providers, the majority of whom are general pediatricians and subspecialty pediatric physicians.

Objectives: Following completion of this educational activity, learners should be able to:

  1. Define the provider’s role in obesity prevention based on the Expert Committee Recommendations endorsed by the American Academy of Pediatrics.
  1. Identify the reason for and importance of screening for, assessing, and managing excessive weight trajectories starting from birth.
  1. Identify the Parenting at Mealtime and Playtime (PMP) strategies that will enhance your pediatric obesity prevention efforts.
  1. Describe the concepts that make PMP an effective way to build sound nutrition and activity foundations through developmentally appropriate parenting techniques.

Needs Assessment/Gap:

Many articles in medical literature discuss the need for prevention and management of obesity, however, obesity continues to be a leading health concern in pediatric care.

Since the 1970’s the number of overweight children in the U.S. steadily increased until after 2000.  Children with BMI >85% at ages 2–4.5 years are 5 times more likely to be overweight at age 12 years (Nader et al doi:10.1542/peds2005-2801 (Dec 2006).  According to the 2007 National Survey of Children’s Health Data Resource Center (http://new.nschdata.org), from 1999-2006 a stable obesity rate of 33.3% occurred in children ages 10-17 with BMI at or > the 85th percentile despite national attention of the problem.

Evidence suggests that obesity-prevention measures need to be taken earlier, in infancy or even before birth.  According to the CDC’s National Health and Nutrition Examination Survey, rates of obesity in children ages 2 to 5 have more than doubled since 1980, from 5.0% to 12.4%.  And once a child sets down the road to an unhealthy weight, it becomes increasingly difficult to change course.  This is what has been learned about obesity:

  • A child of normal weight at age 5 years who remains normal at age 10 years has only a 10% risk of being an obese adult,
  • A child who is obese at 5 years of age has a 50% risk of being an obese adult, and
  • A child obese at 10 years of age has an 80% risk of adult obesity.

Ohio is ranked as the fifth heaviest state in the nation, as nearly two-thirds of the state’s residents are overweight (38%) or obese (28%), according to the Ohio Department of Health, Office of Healthy Ohio.  The prevalence of childhood overweight and obesity has steadily increased over the past 30 years.  In Ohio, 1 in 3 (500,000) children are overweight or obese (Ohio Family Health Survey, 2008).

Of particular importance are children between the ages of 2-5 years old and 6-11 years old; these age groups have experienced an increase in prevalence of obesity of approximately 2.1% and 1.2%, respectively, in the past four years alone.

During the most recent National Health and Nutrition Examination Survey (NHANES) survey in 2006, the prevalence of obesity, as defined by a BMI >95th percentile was:

  • 4% for children 2-5 years old
  • 0% for children 6-11 years old
  • 6% for children 12-19 years old

Since less than 50% of pediatricians calculate BMI in their patients (Ohio Department of Health), educational interventions are necessary.  Periodic calculation of BMI results in early recognition of excessive weight gain relative to linear growth and leads to prevention of overweight children.

Physicians need to regularly monitor growth and provide anticipatory guidance related to parenting, eating, and physical activity in order to perform an important intervention in a child’s life.  Through the promotion of healthy eating habits, exercise and early identification of overweight problems, the physician can have a powerful impact on reducing obesity in childhood.
